My Patient Portal
ACCESSING YOUR TEST RESULTS AND CHART INFORMATION:
Our electronic medical records software provides a "patient portal" into portions of your medical record. After you have provided a secure email address and signed a user agreement during a visit to our office, you will receive an email with your portal user id and password. When new lab results from your physician are available, you will receive an email message at the confidential email address that you have provided to ProPartnersMD.
The portal also allows you to:
• review your medications, diagnoses, and medical history
• request a referral to another physician or healthcare professional
• request a change in your demographic information
The portal is not intended to provide internet-based diagnostic medical services. The following limitations apply:
• NO internet-based triage or treatment requests will be accepted or reviewed. All treatment or clinical diagnosis will be rendered IN OFFICE ONLY, during a scheduled visit with the physician.
• NO emergency communications or services of any kind (example: if
you are experiencing shortness of breath, chest pain, etc.). Any urgent
conditions should be seen by Urgent Care, the Emergency Department, or by
calling 911.
• NO medication refill requests: All prescription refills must be requested through your pharmacist.
