Insurance Coverage

ProPartnersMD wants you to be fully informed about your health insurance and the likely out-of-pocket cost of receiving any of our services.  Our physicians adhere to clinical practice guidelines when recommending health assessments and will not recommend tests that they do not believe are necessary.  Your doctor wants you to know their best advice, regardless of your insurance coverage, and encourages you partner with him in deciding how to proceed.

You are encouraged to ask questions and verify your "routine and preventive" insurance coverage for the CPT codes listed prior to scheduling any health assessment at ProPartnersMD.

If you would like to verify insurance coverage, call the customer service phone number on the back of your insurance card and ask them which of the following procedure CPT Codes are covered for routine and preventive care.  Also make sure you understand your responsibilities related to deductibles and co-payments for these procedures. Then, request a call from our Care Coordinator HERE to schedule your Health Assessment or with any further questions.
